PAYMENT AND INSURANCE INFORMATION FOR BARIATRIC WEIGHT LOSS SURGERY:

During your first consultation, Sterling Surgicare’s administrative team will partner with you to determine any bariatric weight loss surgery medical coverage for which you may be covered.

We will also discuss the Insurance Pre-Authorization process and our payment structure based on your situation.

Patients should be aware there may be out-of-pocket expenses during the course of the pre-and post-operative period. Some employers do not purchase health insurance policies that include bariatric surgery or may not cover some procedures. First and foremost, you should review your medical insurance policy coverage by calling the customer service number on the back of your insurance card.

For bariatric weight loss surgery, ask if your policy covers the “Surgical Treatment for Morbid Obesity” (CPT diagnosis code 278.01.) If they do, ask your insurance company to send you the Medical Policies for this treatment and bring a copy to your initial consultation. This will greatly expedite the pre-authorization of your procedure.

Dr. Adeyeri is an in-network provider with Horizon Blue Cross Blue Shield of New Jersey (Horizon Blue.)

We also offer affordable self-pay options for those patients who do not have medical insurance for bariatric surgery. Please ask us about these plans.

NOTE: Many practices charge program fees above and beyond insurance payment deductibles and co-payments. Our program does not charge extra program fees.

Failure to give 24-hour notice of cancellation of an appointment or not showing up for an appointment can result in a charge of $25.00 on your account. This charge is not covered by your insurance company and is your fiscal responsibility.
